Job Responsibilities:

FACILITATING EXCELLENT PARTICIPANT PROGRAMMING

  • Conducts impactful weekly 1:1 and group sessions with participants utilizing Cognitive Behavioral Therapy and Motivational Interviewing approaches to support participants in achieving their goals.
  • Provides frequent, supportive communications to participants throughout post-release phases.
  • Updates participants' objectives and goals in their treatment plans.
  • Uses assessments to confirm treatment plan goals and progress.
  • Upholds the quality and fidelity of the Concordance Re-entry Model curriculum when delivering 1:1 and group sessions.
  • Prepares for sessions in advance to maximize effectiveness of interactions.
  • Evaluates participant progress and support needed, with the use of assessments and other evaluations as needed.

COMMUNICATING WITH MULTI-DISCIPLINARY CARE TEAM

  • Documents sessions (for individual and groups), including noteworthy participant interactions in electronic record system.
  • Works collaboratively with participants' care teams, providing input on progress and flagging holistic participant needs.

Required Qualifications:

  • Master's degree in Counseling, Social Work, Behavioral Health, or a related field

  • 3-5 years of experience working in mental health and substance use disorders, with equal focus on mental health and substance use treatment

Job Description: Position is responsible for providing substance abuse prevention education programs, outreach, and engagement efforts in the Sumter County community. Also responsible for assisting with community referrals, community coalition building, and advocacy services in schools, as well as with community-based organizations, key stakeholders, government offices, and community leaders.

Job Responsibilities:

  • Coordinates and implements evidence-based prevention programs with older adults, parents, families, youth of all ages, and in schools in Sumter County.
  • Documents delivery of programs, inputs data into computerized collection systems, identifies outcomes, and generates written reports as required.
  • Maintains existing partnerships and develops additional relationships with community partners through participation in local and county meetings or other events.
  • Investigates new materials for agency for youth and adults; makes proposals of new programs and designs "special presentations" as the need arises, such as programs for high school youth.
  • Assists with the maintenance of the organization's prevention resources; maintains inventory of materials and expands current resources; and monitors, records, and responds to requests for information and materials.
  • Partners with and maintains relationships with local law enforcement for Alcohol Enforcement Team (AET), including those offices within the circuit served by Sumter Behavioral Health Services (SBHS).
  • Enters data following community activities to track weekly tasks.
  • Sets up and coordinates events where prevention materials must be present, including set-up, transporting of agency swag, and conducting speaking engagements to share knowledge and educational materials.
  • Travels between a couple of counties when required to disseminate information, schedule and attend "Prom Promise" events and other organized activities per the SBHS County Plan.

Required Qualifications:

  • Bachelor's degree in Human Services, Public Health, or a related field; one year of experience in prevention programming (or a related field); and experience with the development of programs and building collaborations OR a master's degree in a related field
  • Must be certified within three years of hire date as a Certified Prevention Specialist and maintain certification
  • Strong interpersonal and facilitation skills
