Earn up to 26 CME and MOC Part 4 Credit
YAP-P’s no cost Quality Improvement Activity allows eligible prescribing clinicians caring for patients ages 0-25 to earn continuing medical education (CME) and maintenance of certification (MOC) Part 4 credit through consultation, reflection and documentation over a 6-12-month period.
Overview:
- For prescribing clinicians caring for patients ages 0-25
- Providers who complete all required and optional activities will earn up to 26 CME credits and MOC Part 4 credit

How to Get Started
Step 1. Complete the Professional Fulfillment Index (PFI)
To be eligible for any CME and/or MOC credit, you must first complete the Professional Fulfillment Index (PFI), a brief 16-item tool used to assess professional fulfillment and burnout in physicians. Completing the initial PFI make you eligible for up to 0.5 CME credits.
CME Credit Requirements
To receive CME credit, providers must complete at least three individual consultations. Each individual consultation must be about a unique patient case. Once three consultations are complete, you are eligible to receive 3 CME credits. See the section for optional activities for ways to continue to earn additional CME credit.
Required Minimum

Optional Activities for Additional CME
After completing CME and/or MOC credit minimum requirements, you can complete one or all of the following activities to continue earning CME credit.
- Additional individual consultations: earn up to 1 CME credit for up to 7 more consultations
- Cycles of consultations: complete an initial consultation and a follow-up consultation for the same case, with documentation in the QI form; earn up to 3 CME credits per cycle for up to 5 cycles
- Follow-up PFI: complete a second PFI 6-12 months after the first to earn up to 0.5 additional CME credit
MOC Part 4 Credit Requirements
To earn MOC Part 4 credit, providers must complete 3 individual consultations, 2 cycles of consultations with documentation and a concluding administration of the PFI 6-12 months after the initiating administration of the PFI.
For cycles of consultations, completing the QI form for documentation is required for each cycle. A cycle consists of two consultation calls about the same patient case.
